Home from Hospital scheme helps ease patients back into everyday life

For pensioner Kempton Cannon, returning to his empty house after a spell in hospital was set to be a daunting prospect.
The 90-year-old former painter and decorator, of Addingham, was anxious about how he would cope back home living alone.
But now, as a ground-breaking project expands across the whole of Bradford and Airedale for the first time, a package of help and support is available to Mr Cannon and hundreds of patients like him.
Set up a year ago, Home from Hospital is an initiative designed to ease adults of all ages safely back into everyday life after discharge from hospital.

The 90-year-old former painter and decorator was anxious about how he would cope back home living alone.
But now, as a ground-breaking project expands across the Bradford district for the first time, a package of help and support is available to Mr Cannon and hundreds of patients like him.
Home from Hospital is an initiative designed to ease adults of all ages back into everyday life after being discharged from hospital.
It helps with everything from benefits guidance and restarting home care to making sure houses are safe, and that gas, electric and water supplies are in working order.
Patients receive a hamper of food basics and a safety and well-being checklist, plus a friendly face from volunteers involved in the project, which is co-ordinated by award-winning charity, The Carers Resource.
For Mr Cannon, of Addingham, the visits are proving a lifeline.

Stirling University to lead UK’s largest patient care study
The University of Stirling is to lead the UK’s largest ever study into patient experiences and the delivery of frontline health care.
About 6000 patients and almost 1000 nurses and health professionals will take part in the Improving Patient Experience of Care (IPEC) project.
The two year study will also involve academics from Dundee and Glasgow Caledonian universities.
The researchers hope the results can be used to improve patient care.
IPEC’s principal researcher is Prof Brian Williams, Director of the Nursing, Midwifery and Allied Health Professionals Research Unit.

Prof Williams said: “There have been a number of major initiatives in recent years designed to help frontline healthcare staff improve the care experience.
