Guidance on receiving NHS continuing healthcare support

For those who are living with significant medical care needs which will need to be addressed on a long term basis, NHS funding is available in the form of an NHS continuing healthcare plan or grant. NHS continuing healthcare is free, unlike social and community care services provided by local authorities which may be chargeable and are means assessed depending on your income and savings.

It might be very helpful to me.
Mum has been in hospital again all last week but is now back home, seeing social worker today re carers attending.
Mum on a liquid diet now.
I will post when I get time ,
